The review, commissioned by PHE and led by Professor Ann McNeill (King’s College London) and Professor Peter Hajek (Queen Mary University of London), suggests that e-cigarettes may be contributing to falling smoking rates among adults and young people. Following the review PHE has published a paper on the implications of the evidence for policy and practice.
Here we address some of the common questions people ask about e-cigarettes. ENDS may be manufactured to look like conventional combusted cigarettes, cigars, or pipes. Larger devices, such as tank systems or mods, bear little or no resemblance to cigarettes.

The use of e-cigarettes has increased exponentially in recent years, and vaping has become preferred to traditional cigarette smoking among today’s young people and those looking for alternative to traditional tobacco and nicotine products. As use and availability of these products increases, reports have cast more light on the health effects of e-cigarettes. Here, Kenneth Uy, a health coach at Henry Ford Health, clarifies some information about these products and how their use can impact your health.
Upon the selection of the educational track, students may already have normative perceptions of smoking in vocational schools, such as understanding of smoking as an integral and expected practice in these schools and professions [8]. These perceptions may increase S-SM and other nicotine product use, also among students with better academic performance. Student network characteristics and their associations with smoking vary between schools [28].

Such policies are critical in the absence of strong federal regulation of e-cigarettes. As of March 31, 2023, seven states and an estimated 388 counties, cities, towns, and tribes have restricted the sale of flavored tobacco products. However, this leaves a large majority of the U.S. population not covered by such restrictions. E-cigarette sales have rapidly escalated in the relatively short history of the products. Overall e-cigarette sales, as well as the total number of e-cigarette brands, increased by more than 46% between January 2020 and December 2022. Disposable e-cigarettes have nearly tripled in nicotine strength, quintupled in e-liquid capacity, and dropped in price by nearly 70% between 2017 and 2022.
Reynolds’ Velo nicotine pouches, while holding just a 1.2% market share, has had a 25.8% year over year sales growth, as well as up 96.7% for the four-week period. Nicotine pouches have a 5.1% market share with sales up 73.4% over the year. Dr Goel underscored the importance of dispelling misinformation and promoting evidence-based approaches to tobacco cessation.

The program’s curriculum includes a four-part series of lessons, with one set for fifth through eighth graders and another for high schoolers. The particles you inhale while vaping can cause inflammation (swelling) and irritation in your lungs. This can lead to lung damage like scarring and narrowing of the tubes that bring air pockets in electronic cigarette machine in and out of your lungs. Researchers don’t yet know all the effects vaping can have on your body. An electronic cigarette consists of an atomizer, a power source such as a battery,[26] and a container for e-liquid such as a cartridge or tank.
We would argue that further studies with chronic administration of low doses of nicotine are required to clearly evaluate its impact on carcinogenicity. In this line, a study compared the acute impact of CS vs. e-cigarette vaping with equivalent nicotine content in healthy smokers and non-smokers. Both increased markers of oxidative stress and decreased NO bioavailability, flow-mediated dilation, and vitamin E levels showing no significant differences between tobacco and e-cigarette exposure (reviewed in [20]). Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]).
A cigarette is a narrow cylinder containing a combustible material, typically tobacco, that is rolled into thin paper for smoking. The cigarette is ignited at one end, causing it to smolder; the resulting smoke is orally inhaled via the opposite end. The term cigarette, as commonly used, refers to a tobacco cigarette, but the word is sometimes used to refer to other substances, such as a cannabis cigarette or a herbal cigarette. A cigarette is distinguished from a cigar by its usually smaller size, use of processed leaf, different smoking method, and paper wrapping, which is typically white. Not everyone is aware that there is nicotine in most e-cigarettes in varying amounts. Some pods of e-cigarettes contain as much nicotine as one pack of cigarettes (Truth Initiative, 2019).
Currently, e-cigarettes are not mentioned in the Illinois Smoke-free Illinois Act, which prohibits smoking in virtually all public places and workplaces, but some local ordinances do include e-cigarettes. E-cigarette aerosol is not harmless; it can contain harmful and potentially harmful substances, including nicotine and cancer-causing chemicals. While e-cigarettes typically have fewer chemicals than regular cigarettes, researchers found e-cigarettes present their own unique health risks and can increase the odds of chronic cough, phlegm, bronchitis, and asthma. Some cigarette smokers have replaced traditional smoking with electronic cigarettes (e-cigarettes) or vaping, thinking it is not as dangerous. There are significant negative health effects from smoking cigarettes such as cancer, chronic obstructive pulmonary disease (COPD), heart disease, birth defects, and other health problems relating to nearly every organ of the body.
Long periods of dual use of cigarettes and e-cigarettes can result in harms to health similar to, or in addition to, the harms from exclusive use of cigarettes. Nicotine is the primary agent in regular cigarettes and e-cigarettes, and it is highly addictive. It causes you to crave a smoke and suffer withdrawal symptoms if you ignore the craving.
Tetrahydrocannabinol (THC)-containing e-cigarette or vaping products was linked to most EVALI cases. Emergency department visits related to e-cigarette, or vaping, products have sharply declined in recent years, but the CDC continues to monitor reports. In 2022, e-cigarettes remained the most common tobacco product used by high school (16.5%) and middle school (4.5%) students in the last 30 days. And more than a quarter (27.6%) of current youth e-cigarette users say they use an e-cigarette product every day.
In the vaping liquids that contain nicotine, the level of nicotine can vary widely. Prior to July 2021 in Canada, vaping substance nicotine strengths ranged from 0 to over 60 mg/ml of nicotine. Since July 2021, the Nicotine Concentration in Vaping Products Regulations establish a maximum concentration of 20 mg/mL for vaping products manufactured in or imported into Canada. Market size is clearly affected by national laws governing the sale of e-cigarette products. In 2016, the Washington State Legislature passed a law (RCW 70.345) to regulate vapor products.

In other words, just because something is safe to eat doesn’t mean it’s safe to be inhaled. (Duh.) Vaping also seems to trigger potentially harmful immune responses in the lungs.
To date, no e-cigarette product has been approved by the FDA for smoking cessation. In contrast, nicotine replacement therapies (e.g., patch, gum, lozenge) and some prescription medications (i.e., varenicline, bupropion) have been evaluated by the FDA and found to be safe and effective at helping people quit smoking. Manufacturers and sellers of e-cigarettes aggressively target young people.
Little research has been conducted into the safety of e-cigarettes and e-liquids in pregnancy. It is not known whether the vapour is harmful to a baby in pregnancy. E-cigarettes do not produce tar or carbon monoxide, two of the most harmful elements in tobacco smoke.
E-liquid physical properties are also important in carbonyl formation15,47,84,86. The viscosity and density of the e-liquid determine its mobility, capillary action, and delivery to the wick and coil, influencing the likelihood of a dry puff15. Sub-ohm vapes (sometimes called cloud kits) are more advanced vaping devices that are capable of outputting huge amounts of power; if you’ve ever noticed someone exhaling gigantic clouds from their kit, they were probably using one of these! Using a sub-ohm device requires a working knowledge of battery safety and Ohm’s Law, so it’s best to do some reading before investing in one. Reported sales of cartridge products increased from $2.133 billion in 2020 to $2.496 billion in 2021; sales of disposable, non-refillable e-cigarette products increased from $261.9 million in 2020 to $267.1 million in 2021. The prognosis depends on how much nicotine was taken and how quickly treatment was started. If a person is able to survive during the first four hours after poisoning, they’re usually likely to recover. If a person has been severely affected, they may have ongoing seizures or respiratory failure or other problems because of the damage done from low oxygen levels during the nicotine overdose event. Increased levels of nicotine or cotinine (nicotine metabolite) can be detected in urine or blood. Nicotine poisoning or overdose can also result from taking more than the recommended amount of nicotine replacement products (for example, chewing too much gum or dissolving lozenges) or taking too high of a dose of patches, inhalers or nasal sprays.
A 2016 study found that people who use or have used e-cigarettes are less likely to stop smoking altogether. Flavors vary widely, from "traditional" and menthol to watermelon and "lava flow." Some e-cigarettes taste like traditional cigarettes and even mimic the tastes of specific brands. When the user sucks on the mouthpiece, the heating element vaporizes the solution, which the person then "vapes," or inhales. The nicotine content of the liquid can range from "very high" to zero. Refills come in flavors such as cool cucumber, mango, and mint, which may seem natural and harmless, but a single JUUL refill contains as much nicotine as a pack of 20 cigarettes. To inform this work and evaluate the effectiveness of policy, the CDC Foundation is collecting and analyzing information about e-cigarette sales, teen e-cigarette use, and teens’ attitudes about the epidemic.

The 2019 outbreak of EVALI cases emerged against a backdrop of an ongoing vaping epidemic among youth. The amount, quantity and toxicity of substances that e-cigarettes release depend on several factors, such as the type of device, how the device operates and is used, and the e-liquid used. Other compounds that have been detected in aerosols include acetamide, a potential human carcinogen [5], and some aldehydes [69], although their levels were minimal. Interestingly, the existence of harmful concentrations of diethylene glycol, a known cytotoxic agent, in e-liquid aerosols is contentious with some studies detecting its presence [4, 68, 70,71,72], and others finding low subtoxic concentrations [73, 74]. In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Figure 1 lists the main compounds detected in aerosols derived from humectant heating and their potential damaging effects.
Restrictions on importation, packaging and flavourings will come into effect in early 2024 (TGA 2023). Between 2012 and 2022, most Australian jurisdictions have amended their laws to further restrict the advertising and sale of e‑cigarettes (Department of Health and Aged Care 2023b). Recent reports have linked serious lung injury – and even deaths – to vaping. According to the CDC, cigarette smoking causes nearly 1 out of every five deaths in the United States. It’s possible that we don’t yet know all the physical effects of vaping. Numerous studies have been performed to evaluate the safety/toxicity of e-cigarette use both in vivo and in in vitro cell culture.
In 2019, four years after JUUL introduced a new nicotine delivery system coupled with a relentless online, youth-focused marketing campaign, more than a quarter (27.5%) of high school students reported using e-cigarettes. Short-term studies haven't found evidence that exposure to secondhand aerosol from e-cigarettes hurts lung function, with one notable exception. Researchers found that people who were around vaping aerosol showed increases in serum cotinine, which is a marker that someone was exposed to nicotine (an ingredient often found in e-cigarettes). Given the long list of health risks posed by nicotine, more research needs to be done on how this exposure could affect someone’s lungs long-term like it can with secondhand smoke. The significant reductions in smoking rates in the United States, United Kingdom, Australia, Brazil, and other countries that implemented strong tobacco control programs[according to whom?
In fact, they are widely used as alimentary and pharmaceutical products [2]. In an analysis of 54 commercially available e-liquids, PG and glycerol were detected in almost all samples at concentrations ranging from 0.4% to 98% (average 57%) and from 0.3% to 95% (average 37%), respectively [35]. According to a 2018 study examining infrequent cannabis smoking in adults, vaping THC produced stronger mind-altering effects than smoking a similar amount of weed. However, nicotine alone is relatively harmless, and switching from daily tobacco smoking to daily e-cigarette use can be an important step for people to stop smoking completely. However, researchers found that only daily e-cigarette use had a statistically significant effect on smoking cessation rates. Research generally accepts that while vaping can harm the lungs and other bodily systems, its impact is much less than tobacco smoking.
They do contain propylene glycol, but the concentrations are far below levels usually considered harmful. Though this might make the VITAs hit slightly harsher than vapes without this ingredient, it shouldn't pose much risk to your health. But levels are usually low and generally far lower than in tobacco cigarettes. I don’t sit around inhaling lungful after lungful of vape for seven minutes at a time, like I would a real cigarette, so the lingering aura from my habit certainly isn’t hovering for extended periods of time.
In 2014, just under 8% of Wisconsin high school students were using e-cigarettes. In 2018, that number has skyrocketed to 20% (or one out of every five students[i]). They found that while several of the liquids were moderately toxic to the endothelial cells, the cinnamon- and menthol-flavored e-liquids significantly decreased the viability of the cells in culture even in the absence of nicotine. The tobacco cigarette used as a comparator was the Kentucky Reference Cigarette 1R6F (Centre for Tobacco Reference Products, University of Kentucky, USA), which has been designed to provide a standard test piece for scientific studies. It is an unflavoured US-blended king-sized product with a cellulose acetate filter, an aerosol nicotine level of 1.9 ± 0.1 mg cig–1, and a tar yield of 29 ± 2 mg cig−1 as measured by the ISO Intense smoking regime6,56.
Consequently, stricter regulation and a higher quality control in the e-liquid industry are required. The mislabelling of nicotine content in e-liquids has been previously addressed [8, 34]. Of note, several studies have detected nicotine in those e-liquids labelled as nicotine-free [5, 35, 36]. Among the 17 samples tested in this latter study 14 were identified to be counterfeit or suspected counterfeit. A third study detected nicotine in 7 of 10 nicotine-free refills, although the concentrations were lower than those identified in the previous analyses (0.1–15 µg/mL) [5].
Originally, it was thought that vaping might be helpful for people trying to quit smoking. Some e-cigarette labels have claimed that the product had no nicotine when, in fact, it was in the vapor. For this reason, it’s important to use only trusted brands if you vape. E-cigarettes are still relatively new, so their long-term effects are unknown.
Research from The Johns Hopkins University on vape ingredients published in October 2021 reveals thousands of chemical ingredients in vape products, most of which are not yet identified. Among those the team could identify were several potentially harmful substances, including caffeine, three chemicals never previously found in e-cigarettes, a pesticide and two flavorings linked with possible toxic effects and respiratory irritation. "These cases appear to predominantly affect people who modify their vaping devices or use black market modified e-liquids. This is especially true for vaping products containing THC," explains Blaha. The American Lung Association will continue to urge FDA and Congress to remove all flavored tobacco products from the marketplace.

Almost two-thirds of people who use a vape along with support from a local Stop Smoking Service successfully quit smoking. Vaping is not completely harmless and we only recommend it for adult smokers, to support quitting smoking and staying quit.
